Health Care Burnout: A Broader Historical and Societal Perspective

The prevalence of burnout in healthcare has reached alarming levels, particularly considering the commitment and altruism of healthcare providers. A question often asked, and not well answered, is “How did we get to this point?” Looking the broader historical societal journey that has led to today’s environment may be helpful in gaining perspective on the current situation, and may suggest changes in perspective that may allow healthcare providers to regain the proper relationship with change that can be more supportive and productive, and allow them to form methods to address burnout.

Where are we now? An Update on the Stark Law

The past several years have seen a number of important developments in the way that regulators are interpreting the Stark Law. Enforcement agencies and False Claims Act whistleblowers are increasingly focusing on technical violations of the law, including whether compensation to hospital physicians reflects commercially reasonable relationships; improper stacking of payment arrangements between hospitals and physicians; how hospitals can pay for call coverage; different ways to establish fair market value; whether referrals can be tracked; and whether funding and paying incentive pools improperly takes into account the volume or value of referrals. Meanwhile, with payment models increasingly shifting towards value- and quality-based payments, a thorough understanding of how to structure physician compensation models in compliance with a self-referral law that is based primarily on fee-for-service reimbursement has never been more important. This session will explore how the Stark Law has been evolving in these and other areas, review its application to quality-based compensation models and discuss key False Claims Act cases filed against hospitals and physicians based on alleged Stark Law violations.

Telemedicine the future is now.

Telemedicine-The Future is Now! The Essentia Health Telehealth program provides over 5000 visits annually, involves over 30 programs and is operational in over 50 Essentia Health clinics across northern MN, WI and ND. This program will introduce cugfrrent infrastructure and revenue models used in telemedicine clinic and provider settings. Return on Investment for the telehealth providers and the originating patient sites will be reviewed. The 2016 MN Telemedicine Law and MN MHCP guidelines will be contrasted with Medicare payment guidelines to present eligibility and reimbursement information. Benefits and challenges that the clinic settings experience on the provider and patient sides will be explored, along with internal processes and costs. Actual telemedicine program examples will be presented to highlight involved processes such as scheduling, coding, compliance and nursing roles. An example of a Work Flow Process Map will be shared showing the many department interfaces.
